Follow these steps to submit your request:
Open the Zofran request link:

Log in to your Polly account using the following options:
Enter your Polly email and password,
Sign in using a Magic Link,
Create an account or Reset your password if needed.
Review your treatment then click Next.
Modify your plan (optional):
Click Modify Plan to select the number of tablets.
Click Save and Continue.
Proceed through the steps:
Click Next.
Confirm or update your shipping address:
Your current address will be displayed.
Enter a new address if needed.
Click Continue.
Review your order and enter payment details:
Add your credit card information, or
Use Google Pay or Amazon Pay.
Submit your request:
Click Submit to complete your order.
